Assessment & Diagnosis

When you first see your Mental Health Provider, he or she will perform an initial checkup called an Initial Assessment.

This Initial Assessment can include.

  • A physical check, such as obtaining your height, weight, blood pressure and such things.

  • Personal Medical, Physical, and Social History, including questions about your family members.

  • Discussion about your symptoms and reason why you are seeing the provider.

Based on this assessment, your Mental Health Provider will then make a diagnosis if it’s clear and precise. If the diagnosis is unclear your Mental Health Provider may request more testing or request more visits to validate and confirm a diagnosis.

Here at Modern MOjO we treat every Initial Assessment of a patient as a new opportunity, we do not automatically apply other providers previous diagnosis to you. We use them as a guide or reference or a piece of the whole puzzle. If your Mental Health Provider finds that a patient no longer meet the criteria from another providers previous diagnosis then we will not automatically assign it to any persons.

We do not copy and paste! We Validate and Verify!

Once the Initial Assessment and diagnosis/possible diagnosis are complete, YOU and your Mental Health Provider will develop a treatment plan designed with your specific needs and goals in mind. The decision is ultimately up to the patient, together you will come up with a plan.

The plan is different for everyone, it can include such things as medications, treatment, therapy, counseling, exercise, testing, goals, and much more. It’s important to note that not all patients and not all psychiatric problems require medicine.
